Michael Cox, PT, DPT, OCS | Carrollton Clinic Director

Michael Cox, PT, DPT, OCS graduated from The University of Texas at El Paso with a Master of Physical Therapy degree in 1999 where he was named the “Outstanding Scholar” for the graduating class. He has a Bachelor of Science in Healthcare Sciences from University of Texas Medical Branch in Galveston and a Bachelor of Science in Exercise and Sports Studies from University of Texas at Arlington. He received his Doctor of Physical Therapy degree from Hardin Simmons University in 2010.

Michael has practiced in orthopedic and sports medicine physical therapy settings since his graduation. In June of 2003, he was certified as a clinical specialist in orthopedic physical therapy by the American Board of Physical Therapy Specialties of the American Physical Therapy Association (APTA). In 2002, he won the Dorothy Briggs Memorial Scientific Inquiry Award from the APTA for research on the effects of different types of warm-up prior to stretching the calf muscle.

He has been working with the physicians in Carrollton since 2000 and has been Director at the Carrollton location since 2003. Michael successfully completed a CARF certification in 2003 for the facility.  He has been instrumental in developing rehabilitation protocols and new programs at the facility including Aquatic Therapy. He has treated athletes and sports medicine injuries his entire career emphasizing functional exercise progressions, corrective exercise techniques, and manual therapy.
Outside of work, Michael enjoys spending time with his wife and two daughters. Michael is a die-hard Longhorn fan and loves the Texas Rangers. He enjoys outdoor activities including fishing, camping, and hiking. top of page

Shelley graduated from Texas Woman’s University with a Bachelor of Science in Nutrition in 1998 and a Master of Science in Physical Therapy in 2000.

Shelley has worked in a variety of physical therapy settings, from orthopedics to hospital acute care, nursing home and home health care. Shelley has been employed at the Denton facility for 7 years.
Shelley’s clinical interests focus on treating the local athletes with acute sports injuries and helping post-surgical athletes return to sports. Shelley also enjoys treating patients with joint replacements and shoulder dysfunctions.

Cindy S. Murray, PT, CEAS has been practicing as a licensed physical therapist since 1992. She graduated from Texas A&M University in 1990 with a BS in Health Education, and then received her BS in Physical Therapy in 1992 from The University of Texas Medical Branch at Galveston.
Cindy has practiced in Orthopedics and Sports Medicine physical therapy since her graduation. Furthermore, she has been active in the Industrial Medicine component of rehabilitation. In 2002, she earned her certification to become a Certified Ergonomic Assessment Specialist. That same year, she was awarded the WorkSTEPS ACE Award for outstanding Achievement, Commitment, and Excellence in the WorkSTEPS employment testing program. This award is presented to one provider in their network which consists of over 600 locations throughout 48 states.
Cindy has been involved heavily in educating providers regarding Workers’ Compensation in many ways. She authored the Workers’ Compensation Guide for Texas Physicians, 4th edition, for the Texas Medical Association, and was involved with the Texas Medical Association, teaching seminars for a number of years. She was a speaker for the “Impairment Evaluation under Texas Workers’ Compensation” training courses, and for the seminar “Mastering the Workers’ Comp Obstacle Course”.

Cindy has been the Director of Rehabilitation at the Plano location since 1998. In 2002, she expanded the rehabilitation department and opened the Frisco location. She has grown the locations from a one therapist facility to a 8 therapist facility.

When not at work, Cindy enjoys spending time with her husband and two children. Her daughter is an avid equestrian and her son plays baseball year round. Cindy has a horse that she competes in Dressage, a type of English riding.
